A 90-minute comprehensive evaluation created to understand what you are experiencing, what may be contributing, and what kind of care makes sense from here.

Together, we explore your mental and physical health history, current medications and supplements, previous treatment, sleep, nutrition, hormonal and gut health influences, stress patterns, life experiences, and personal goals.

Your Foundations Session may include:

  • Comprehensive psychiatric evaluation
  • Review of available records and laboratory results
  • Medication and supplement review
  • Assessment of nervous system, lifestyle, and whole-body influences

WHY R4

A diagnosis can name a pattern. It cannot explain the whole person.

When symptoms return despite your best efforts, it is easy to wonder whether you are the common denominator in every treatment that hasn’t fully worked.

What looks like dysfunction may actually be adaptation — a nervous system that learned to protect you and continued doing so after the original threat had passed. That is not a character flaw. It is a response that deserves to be understood in context, not simply managed.

R4 begins there: understanding what shaped your symptoms, what continues to reinforce them, and what may help create meaningful, lasting change.

I built R4 because I knew what it was like to seek help and still feel that something was missing.

I’m Vanessa—a board-certified psychiatric nurse practitioner, Marine Corps veteran, and, for years, a patient within the same healthcare system I was trained to work in.

My path into nursing began with my mother’s cancer. Watching her navigate oncology taught me how much can be lost when treatment focuses on the condition but loses sight of the person living through it.

Across more than a decade in crisis, geriatric, child and adolescent, inpatient and outpatient behavioral healthcare, I continued to see the same pattern: people were surviving, but not always recovering. Medication could reduce symptoms. Therapy could create insight. Yet many still struggled to reconnect with their health, purpose, relationships, and themselves.

My own search extended beyond the exam room. Nervous system work, breathwork, lifestyle medicine, community, and personal healing showed me that meaningful recovery rarely comes from one intervention alone.

Years later, ketamine-assisted therapy added another layer. It was not a cure, but it helped create space to process what insight alone had not resolved. Rather than changing what I believed about healing, it affirmed it: the right treatment can be powerful when it is used thoughtfully, at the right time, within comprehensive care.

These professional and personal experiences became the foundation of the R4 Method: Release, Regulate, Reset, and Rebuild—a framework created to move beyond managing symptoms and toward restoring the whole person.

We do not measure success by how long someone remains in treatment. We measure it by how confidently they move forward without needing us.

Ketamine-assisted therapy is administered under direct clinical oversight in every session, with a licensed psychiatric nurse practitioner present throughout. This is not an unsupervised experience. It's a clinically monitored medical process, held inside a rigorous and deeply human standard of care.
